knowing the sort between public and private is done on the folder level (not individual photos), could I use direct links in the public and family directories instead of real copies?
I don't want anybody to be able to see all the images and I'm not sure of what a link can do with <directory> access

Yes you can keep all your photos in a totally different place, and access them with links stored in the appropriate directories. There is a setting for Apache that can control whether or not it will follow links. One picture may be linked into several other directories.
