Checking Alt+F3/Advanced/No border on any of the windows could help, possibly also Alt+F3/Advanced/Special settings/Preferences/No border . The latter could be also used to override whatever is causing this problem if you cannot find the real problem, if you manage to explain the user how to create a rule that would force 'no border' disabled on all Firefox windows (good luck with trying that over the phone).
The oddity continues... but. in a good way I guess. We tried a few things... log out, log in etc, and Firefox still had no window decorations (I was watching most of the user actions via the Skype share desktop thing so I was able to actually see what the user was doing). At some point, the user needed to step out for a couple of hours, so Firefox was shut down, but the computer was left running. When the user came back and restarted Firefox.... everything was back to normal. So, we never got to try the Alt+F3 options...
