The insurance companies can negotiate big discounts, but for those people who can't afford insurance they have to pay list price, which can easily be several times what the insurance companies charge. As always, those who can least afford it, have to pay most ... (I can't defend a lot about the NHS, I think we've gone the other way where we won't let people pay who can, but at least care is available to everyone, and it's reasonably decent care.)
