I regularly use Skype and Ekiga (with freecall.com or ekiga.net providing my SIP servers) for PC to PC, PC to Phone, and Phone to PC calls... and conference calls. Skype is probably the easiest (in my experience) when you have a mix of PCs/OSes, land lines, mobiles etc.
