What is really missing is the URL where the original form can be obtained so that I/we can see for ourselves what happens when I/we open it in out copies of LibreOffice.
For example, I am running LibreOffice v3.6.3.1 and so expect to get a different result to you.
So, what is the URL for that form?

Not sure, in general, what has happened but I've almost given up on Libreoffice and gone back to OpenOffice.

I see that OpenOffice is still available as a freebie. I don't remember the reasoning, but why was OpenOffice dropped and replaced with LibreOffice?
