That's one reason I got out of commercial programming. One time delivering what they had actually asked for because I had a back-channel to the customer had me physically assaulted by the marketing manager. That programmers are not given the tools, opportunity and motivation to "Get Things Right" is a management decision, and its often driven by marketing and featureitis. Project managers fear scope creep more than anything. BTDT. I've had young programmers on technical forums take a 'well if you're so smart why don't you code it yourself' attitude. There's a reason, and its not about incompetence.
