In my case, it is software. It worked with 10.3, stopped working with 11.0, and works fine with 11.1. Same hardware on all tests.
Furthermore:
It works if I use it from inside a vmware 11.1 guest on a 11.0 host, and export it via NFS from 11.1 to 11.0.

They had issues with Windows too, the disk was replaced, the provider tested it with Windows and it was fine. Now I reformated as XFS and created a virtual disk for Virtualbox on top of it. The VM got I/O errors after copying 4GB of data. :s Two disks in a row are unlikely to be faulty. I'm running 11.1 so it can't be the same issue you mention as it works on 11.1 in your case.
