I do not understand the significance of staying in sync with the RPM database.
Staying in sync is a fundamental requirement of the package management system. Break it, and break your installation. Don't mess with it under penalty of irreparable problems. Zypper and YaST know how to use it properly. You need not be concerned with "keeping in sync" if you stick to using them according to instructions. If it gets broken, it may be possible to fix it using "rpm --rebuilddb", but don't count on it if you've been using a file manager or rm to meddle with system files.
