Is there a way to convert a PDF document to a human readable format, add the required text and then re-PDF the form?
Yes, but it requires a lot of work. You'd have to OCR the form, hope all lined up correctly and then re-pdf the form. An easier solution is to simply open the form in OOo or GIMP or whatever and add text blocks where you want them. You may need to save the form in either TIFF, JPEG, PNG or another suitble format first.
