Does /etc/asound.state start with "start.CA0106"? I would also clean out entries for the other card. Reboot, and if it reappears, recheck the BIOS setting. The kernel may see it, but I don't know how with it disabled in BIOS. As root, run lsmod | grep ca0106 to see if snd_ca0106 and its related modules are loaded.
