chkconfig apache after a fresh boot says "apache on"
Check what 'chkconfig -l apache' says. If it doesn't show at least 3:on, do a 'insserv apache' and then a 'rcapache restart'. At the next boot apache should start automatically. BTW, you can also check this by doing 'init 2' and then 'init 3'. When going from 2->3 apache should get started.
