I have upgraded from 9.3 -> 10 -> 10.2 -> 10.3 -> 11, the most serious issue was a reiserfs bug causing system hangs. If you're fairly experienced with linux, it's the best option in my opinion (you need to update RPM first). Of course, a backup never hurts. You may need to adjust the configuration of some programs, servers and the bootloader, in almost every upgrade I had a fully working system in ~5 hours.
