I have copied the relevant section of my XF86Config file below. You'll see that my "Screen" inputs are designated "Screen 1" in the config file. I'm not sure how to add the comment-- "Screen1" to the "ServerLayout" section.

I just had a look at the /usr/X11R6/lib/modules/drivers and saw a file named radeon_drv.o. So setting Driver to radeon instead of r128 may works better for a radeon equipped card.
