The only time I've seen this happen is when I have a filter active and the new messages are filtered out. The count of unread messages is non-zero and you can hit space or N or + all you want, it just sits there happily showing you the same filtered message.
Could that be what you're seeing? I certainly do have messages filtered into folders, and some SPAMS deleted. I did find an anomaly this morning as we had a power failure last night. As I was booting the system this morning, KDE complained that I had another instance of Kmail running. (Which was probably a lockfile that got left around).
