You can convert the DocBook 4 source into 5, see here: http://doccookbook.sf.net/html/en/dbc.structure.db4-to-db5.html This would make it easier as you have to deal with only *one* version.

... and even with the docbook4 stylesheet path the man page is mangled.

Strange. Do you have a link to your DocBook XML source?
