Weird. Is the filesystem for sure ext4? Default is still Btrfs, so unless ext4 is explicitly selected I'd wonder why fsck.ext4 is being called at all. Seems excessive but harmless. Recreate the lost + found directory and chattr +i, reboot and then check the journal for "permission denied" or some such.
