FWIW, I've run into exactly that problem before. Swapping out the adapter with a different one fixed the issue.
11.4 has a probably related problem, cramming all output into the top 1/8" or so of the screen, but at least I can get to a tty to kill X and shutdown. WinXP doesn't seem to care whether I use an adapter or not, working apparently fine with straight DVI cable, or to legacy 15 pin via adapter. -- "The wise are known for their understanding, and pleasant words are persuasive."
