That is never-ending and will not provide a definition that satisfies you. Equipment not available, testers not available, failure to file bugs, and on and on .... will always lead to recent comments about "failure to test" and pushing commits w/o proper testing and .... Which you have experienced in the last several days. In *this* respect a ten-year-cycle would not be an improvement, but only *different*.
