I use http://www.youtube.com/watch?v=xMLMHJjMN0o lately to test if Youtube works. On my latest 13.1 installation at least, the feature video looks like it's trying to run in 4 or 8 color mode in 4:3 aspect ratio. On same system, it works as expected both in WinXP (Flash not installed) and Fedora 20 (Flash not installed).
installed: flash-player-11.2.202.310-2.1.6.i586 flash-player-kde4-11.2.202.310-2.1.6.i586 konqueror-4.11.2-1.1.i586 MozillaFirefox-25.0-1.1.i586 pullin-flash-player-12.3-4.1.2.i586 seamonkey-2.22-1.1.i586 xf86-video-intel-2.99.905-1.1.i586
Removing *flash* solves the problem.
Same for everyone? Should Flash/pullin-flash not by default be left uninstalled?
How are those who need Flash available for non-HTML5 sites supposed to deal with this? Does FlashBlock need to be paired with Flash installation?
I tested your Youtube link and the video works for me.

I am using the radeon driver for the old AMD graphics card in my test laptop. Beside the standard installation (os 13.1 RC2) I added the packman-factory-repository.
On a different 13.1/KDE4.11.2 system, with rv200 Radeon AGP gfx, without Flash installed, http://www.youtube.com/watch?v=xMLMHJjMN0o was working fine. Then I did zypper in flash-player and rebooted. Afterward, that same URI on reload painted a black player window initially, which then turned white, disappearing into the background. No sound played either. About plugins reported Flash to be installed. I closed FF and installed flash-player-kde4, reopened FF, the video plays, but about plugins reports no enabled plugins found. Right clicking the video provides no opportunity to discover what enables playing the video. :-p
My OP described an installation on a system I do not own, so it may or may not be available on which to follow up.
On still another 13.1/KDE4.11.2 system, with i845G gfx, without Flash installed, same URI works in FF ESR17. I closed FF, then zypper in flash-player flash-player-kde4, and restarted FF. It now behaves poorly, almost exactly as described in my OP. Instead of a appearing to be 4 or 8 bit colored 4:3 video, the 4 or 8 bit color is limited to the left half of the wide aspect video, while the right half is all black, plus about:plugins does report Flash 12.2.202.310 is installed.
I have a similar problem with FF on opensuse 12.3, but with no all the youtube videos. When it do'nt play I get a message I must to install the flash plugin. A time ago I played all the youtube videos with Chromium, without problems, but now I get a black screen with the message "Something is wrong". The only way to play all the youtube videos on opensuse 12.3, is with Konqueror. In Firfox I have installed the flashblock plugin, and before installing it hatte problems with some youtube videos.
