From my experience is this the real problem.
In other words: I think the real problem with any kind of firewall setup is to make the basic concepts behind understood by the user who must make a reasonable decision for his particular environment. Please no knee-jerk proposal to "autodetect his particular environment and setup 'the right thing' automatically" ;-) I think this is a really hard usability problem.
