I tried to reproduce with dolphin from K:U:A, no luck
Tested with: mkdir ~/test cd ~/test touch 1.jpg 2.jpg 3.jpg a.jpg b.jpg dolphin . select all, F2, leave '#' and click on 'rename' (repeat a couple times)

That looks like the modal dialogs appear in a wrong order, so only one of dialogs can be interacted with, but not the topmost one.
In dolphin 18.04+, https://phabricator.kde.org/D9836 completely changed how batch renaming is implemented, but it requires newer KIO so we can't backport it.
