I have noticed this problem on all the blogs with this spam plugin, and it is not a new issue. Doing a hard refresh on the page usually resolves the problem, as it sets a new math question for you.

I experienced this differently. Refreshing the page ("Upper Case" Reload Page) didn't help in several cases I experienced. Actually, I think you're right. You might have to start a whole new session to get it to work. It's been a number of months since I have seen this happen, since I don't comment a whole lot on the blogs.

In any case, I agree that it can be extremely annoying to end users. I thought that the issue was already known, so that is my mistake for not reporting it myself last fall.

Matthew, this is not your fault - this is kind of the fault of some hundred people including myself. I experienced this for the first time at least a year ago and I know from several people who know that it doesn't work but haven't opened a bug yet. I suspect the Threaded Comments plugin. However, I came across a plugin called Anti-Captcha. It uses javascript to create a hidden input field with a hash that will be compared to a server generated value. Since bots don't generally handle javascript, this serves the same purpose as any other captcha system.
