In all the cases of this problem I was debugging so far it was a userspace problem and we need to find out which userspace program is guilty. So please run with that change (possibly use: fuser -vm / 2>&1 | grep F | tee /tmp/cannot-umount so that you have the output after reboot) and report the output after you see the problem.
