There is no way of repairing the stick without using another linux computer and e.g. deleting partition 4 on the stick or rewriting the image. This is especially bad if you want to use the stick as a rescue medium :-) The COW file seems to sporadically destroy itself during shutdown (I also often see I/O errors flashing by on shutdown - related?). The issue can be triggered by manually corrupting the COW file ("echo > /read-write/.clicfs_COW; sync; reboot -f"). IMHO the system should just boot without the COW file (in "LiveCD Mode"), then the issue can be fixed from the running system. This was also present in 11.4 if I remember correctly. I can boot a LiveCD on USB once, and then it will not work again forcing me to 'dd' it again if I need it.
