After some investigation I think I can pinpoint the problem to an error in the order in which the tasks during initrd's run_all.sh are executed. As far as I can tell, for on-demand firmware loading you need udev. But this is started after loading the modules. So I manually modified the initrd for testing to load the qla2xxx after line 34 "source boot/04-udev.sh". Then it worked correctly for my needs. The file /lib/udev/rules.d/50-firmware.rules is missing from the initrd.
