The configuration of extra mice buttons through xorg.conf is too complicated for an average user, so many users simply leave the extra buttons unused, which is not making Linux more user-friendly in their eyes. The same applies to multimedia buttons of modern keyboards. In Windows one has e.g. Logitech drivers that allow for a simple and quick configuration. Presently there are serious advances in Linux configuration tools. For example see the keyTouch project (http://keytouch.sourceforge.net/), which embraces most of the keyboard models on the market. However, I was answered that "Configurations like that are better done on the desktop systems The big advantage there is that you can do changes on the fly which sax2 can't do because it writes persistent data to the xorg.conf", so I had to resubmit a feature request for KDE. As an end user, I feel that the feature is missing. But of course I can not decide whether it should be added to Sax2 or KDE. The decision is up to developers. But this is not so important as to have the configuration tool in any form, let alone in the form of KDE applet. Are you talking about configuring the keyboard and mouse themselves, i.e. that the proper hardware model is selected, or about configuring their functionality. If the sooner, that's handled by X. If the latter, then I don't see why you mention xorg.conf, as that has nothing to do with it - you can configure the play key to do something in Amarok by simply assigning that key to an action in Amarok's configuration (assuming keyboard itself is configured properly). The handling of such events and its configuration is not part of the xorg.conf because this is something handled dynamically and belongs to the desktop. That was what I tried to explain in comment #2 basic configuration of for example multimedia keyboards is possible with sax2. Setting up event driven actions like play or surfing aren't. I mentioned xorg because AFAIK to assign an action to an extra mouse button one currently has to i) install imwheel, ii) edit xorg.conf and iii) edit .imwheelrc, see e.g. http://forall.ru-board.com/Demetrio/lor/imwheel.txt . This is not too convenient, especially as compared to the way provided by a Logitech driver in Windows: just start the configuration dialog from the Control Panel and choose the actions for each mouse button.
