This can be observed when using yast (ncurses). Focus is lost when <shift>+<tab> is pressed. <backtab> support is correctly provided in the *.keytab files. However, <shift>+<tab> is instead used switch (backward) between the items on Konsole's Tab bar (the 'Close tab' button, tabs (all tabs form a single item) and the 'New tab' button). Note: this does not make any sense when the New/Close tab button are not shown. I found no way (in the settings) to disable this keyboard shortcut. Configure shortcuts does not list 'Cycling through tab bar items' as an available action. I suggest Konsole passes <shift>+<tab> to the terminal by default.
