When the input is grabbed via ctrl-alt-G, both the key focus and the mouse are grabbed to the qemu window. Then move your mouse, and you'll see the pointer stops moving at some point suddenly. It starts moving again when you move the mouse backward to some point. It seems that the X mouse pointer isn't actually fully grabbed but it's only invisible. When the (invisible) X pointer goes over the qemu window, qemu loses the mouse input, thus the mouse pointer in qemu window freezes. When the X pointer comes back to the position of qemu window, the mouse pointer in qemu starts moving again, too. The problem isn't seen in SDL UI. It grabs the X pointer, and after releasing the grab, X pointer re-appears at the same place it was grabbed. The behavior might depend on DE.
