The MacBook contains the following sound card: 00:1b.0 Audio device: Intel Corporation 82801G (ICH7 Family) High Definition Audio Controller (rev 02) Using alsamixer and decreasing or increasing the soundlevel on the left side does not change anything. On which mixer element are you referring to? I am referring to the PCM mixer element.

Which output are you hearing? The "Center" channel seems to do nothing at all. The internal speakers volume is affected by the "Front" and the "Surround" mixer elements. The "Front" channel is handled correctly by the balance slider. Which has the following relevant versions. kdemultimedia3-mixer-3.5.8-9 alsa-1.0.15-2 kernel-default-2.6.23.1-6 BUT: # cat /proc/asound/version Advanced Linux Sound Architecture Driver Version 1.0.14 (Fri Jul 20 09:12:58 2007 UTC). So one is 14 and one 15. If I set the kmix main channel to "Master:" Then the master volume goes down when moving the right-left slider from the center to the right. (Moving it back does not increase the volume again.) If I move the slider from the center to the left nothing happens. Is that a KDE bug? If I set the kmix main channel to "PCM:" Then moving the right-left-slider to the right does nothing and moving it to the left mutes the "souround" channel. (This is not reflected by alsamixer or kmix, but that is what I hear.)
