This is not working on a dm-raid partition, but I don't know if it working with /disk/by-id ? I hope the Log can help but I found so many other problem that all together is a total broken Installation :(. From what I can see in the log installation went fine, fstab looks ok. No idea why reboot failed, it should not be connected to yast2-storage related stuff. But I see that there is a different bug for the boot problem anyway.
You used an installation system with libstorage version 2.21.5 and kernel 3.0.0-4. The System you installed contained libstorage version 2.21.6 and 3.1.rc5. The System used during installation must not differ from the installed system otherwise strange side effects can occur since until first reboot different code is running than after reboot. Nevertheless I found a problem in the log file that caused that you were not able to switch to mount-by-id for your BTRFS root filesystem.
