Trying fam - which doesn't work even out of the box with systemd (filed a bug), made me think: - fam isn't maintained. - gamin hasn't been especially stable - inotify, however, has been around a while and seems to work. Can fam/gamin support be removed from KDE and replaced/supplemented with inotify? Our startkde.suse.sh script still sets USE_FAM, for what that's worth. Furthermore, as i mentioned already, it is needed by remote FS's like e.g. NFS. You can even contol the method via kdeglobals for which default on local FS's is INotify.
