This happens with an existing user, with some GB of mails. (With a new test user, KMail doesn't eat up the CPU - but the test user does not have any mails.) I'll happily help to debug this - just tell me what you need. KMail 4.10.95 and Akonadi 1.10 from KDE:Distro:Factory work fine on my 12.3 system. And as I said, 4.10.95 works fine here. So could you maybe try to update your KDE to the versions in KDE:Distro:Factory? IMHO it's not worth debugging this if it is already fixed. On the other hand, which Akonadi database backend are you using? As a workaround, I downgraded kdepim to the packages from KR410 (rebuilt for Factory in home:cboltz:kde410). This gave me a working KMail again. (I didn't need to downgrade akonadi.) I still get the same problem with KDE 4.10.95 from Factory, and also when installing the latest akonadi and kdepim packages from KDE:Distro:Factory (which has KDE 4.10.97).
For the records: still happens after zypper dup --from KDE:Distro:Factory (to avoid a mixup of packages from different repos)

Ignore this notice - the patch was in kdepim-runtime (now in upstream code), not in the kmail package.
To make things slightly more interesting, akonadi in Factory now requires kdepim >= 4.10.95, so I'm just building akonadi-runtime in home:cboltz:kde410 to (hopefully) get a useable KMail again.
This gave me a fast kmail - but in read only mode. I could read mails, but not move them to another folder or delete them. After removing the duplicate outbox, KMail works again :-) (If you are interested in more technical details, have a look at the upstream bugreport.)
