It even gets confused inside 12.2 without reinstalling the operating system. Do you have more than one audio card? (My laptop has HDMI, which I never use but it's the default after system installation, it has internal Intel audio where the speakers and the analog headset outlets are connected, and last not least I use different USB headsets from time to time). I'm not sure how the tray icon applet decides, which audio device to display, but obviously it can display only one of them. Actually I'm not even sure how it should do it in a perfect world, if you have configured some sounds on the speaker (e.g. phone ringing) but others in the headset (phone conservation). But when things are screwed up it has helped to disable all but one audio device (e.g. in pavucontrol application) and restart the session.
