It seems some BIOS are disposed to change boot priority/BIOS disk order at the slightest provocation that may or may not come from actually entering BIOS setup or changing anything in BIOS setup, but might come from any transient or worse problem with BIOS HD0 during POST, or from HD replacement. Once this happens, /boot/grub/device.map may no longer be correct.
