Using mouse and/or keyboard via USB, I get regular lockups ... no reaction from mouse/keyboard, unloading the ohci-hcd hangs indefinitely, so does lsusb, cat /proc/bus/usb/devices, and any other access to the ohci subsystem. ehci/high-speed (2.0) devices still run. The hanging processes cannot be killed (-9); shutdown (umount /) thus fails. Similar things happen with certain usb 2.0 devices, e.g., a smart card reader that switches from usb 2.0/ehci to usb 1.1/ohci when an sd-card is inserted, and then kills the ohci. The same card reader giving read errors after ~400MB when I got it to run on ehci. A USB-serial converter is also apt to kill the ohci on plugging in, or a few minutes later. Using an ps/2 keyboard, the death occurs less often, but still does (mouse, etc.). Using certain ports, sometimes only part of the ohci crashes, i.e., the usb-serial hangs, the mouse still works until I try to unload the ohci-hcd (probably due to the two different OHCI controllers as listed below). The bug seems very similar to the above mentioned #214247, where the same symptoms were detected with an nForce4 (430) chipset.
