Isn't the driver contained in your initrd? That is why blacklisting wouldn't work. Please attach your initrd contents: zcat /boot/initrd-`uname -r` |cpio -t How do I do that?
Just in case, could you normally boot without the card, modprobe all of /lib/modules/`uname -r`/kernel/drivers/pci/hotplug/* and plug the card in? Does lspci contain the device?
