It was actually a recent nfs-utils update that broke this. I forgot to add this fix when I made an nfs-utils update for 11.4 a couple of months back. Oops. I've taken more definite steps to make sure I get it right if there is a next time. My apologies. Just "sudo chmod 711 /var/lib/nfs" and it should be OK.
