File manager mode? Details view? "Focus" is not the same as "selection", this confused me a bit. Keyboard focus is the dashed outline around the single item that keyboard commands like Enter act upon, selection is the coloured bar around the items(s) that menu/toolbar actions would act upon. ctrl+enter selects the focused item. But I suspect that this is changed behaviour in Konqueror's new file management part compared to what you are used to. In Konq in file manager in Details view, a click anywhere in the Name column will open a file, to make the clickable area consistent in size (short filenames were hard to click). The new part has a system for allowing (multiple) selections in a single click interface without using the keyboard that was impossible with KDE 3, and you can use it to _select_ a file without opening it, eg for renaming with F2. Hover the icon and click it once - it is selected and focused. You can now add other items to the selection in the same way, press F2 or delete.
