You are using totem in KDE4?
In KDE4, phono-gstreamer backend fully uses gstreamer, thus, totem can't acess gstreamer to play sound.
A workaround is installing xine backend of phonon.
sudo zypper in phonon-backend-xine
then, systemsettings->media->set the backend to xine and select every output to PulseAudio.
open Yast->Sound->Other->PulseAudio->enable it.
Retart the computer and see if it works for you.
