Question: The boxes I'd reported having trouble with all had root "/" on LV-on-RAID-10. To test this on those production boxes again will take a little time. I *do* have a less-critical, more readily available, box that is similarly configured, but rather than RAID-10, metadata_v1.2, it's "/" on LV-on-RAID-1, with metadata_v1.0 Looking at the script, IIUC, neither the RAID class nor metadata type should matter. Is that correct? Will a test on the -RAID-1, metadata_v1.0 box be sufficient? or at least informative?
