Partly, this can be caused by the fact that the filesystem is changed while the fsck runs (so fsck has an inconsistent view of the metadata), partly by the fact that filesystem is not "cleaned up" while it is mounted and some state is kept only in memory in kernel structures. In your case, there are deleted but still open files in the filesystem.
