Also make sure that alsa-plugins-pulse package is installed, and the environment variable $ALSA_CONFIG_PATH is set to /etc/alsa-pulse.conf. Then remove once ~/.config/pulse/*, and restart. If the problem is seen again, you can try to restart PA with a verbose log. For that, disable autospawn in /etc/pulse/client.conf (set autosspawn=no).
