Not a direct answer to your question, but for HDMI input I currently use one of the cheap (~20€) HDMI-to-USB-2.0 dongles which started to appear a few months ago. Works out-of-the-box as these are UVC compliant.
Yes, tried that too but AFAICT you cannot control the resolution.
