There are different types of updates. By default, YaST Online Update and Zypper only look for "patches". The other updates that the openSUSE Updater refers to are updated packages from other repositories.
You can list these by using "zypper lu -t package".
That display is rather confusing. It is not at all clear if the version number is the version that is installed or the version that is available.
