No problems with normal boot. Clearly one or other USB subsystem not being
detected on a Intel(R) Core(TM)2 CPU 6700 @ 2.66GHz running Linux
2.6.18.2-34-default x86_64.

The faulty safe mode boot log messages reads

it would seem the device mapper is not there
