No, the first incantation is correct. I may be way off here because I don't use fetchmail, but I would think the access.db on your machine wouldn't help. The mail has already been accepted by the ISP on your behalf. The MTA that matters is the ISP's not yours. Yes, This is the question ... I believe that access.db modifications are correct but fetchmail don't use sendmail to fetch my mail from my ISP, but in fetchmail's faq it's said that this is the correct way.
