Did you also test it in 1024x768 mode while in sax2? What happens if you disable the 640x480 mode? You are of course aware that you can switch between selected modes with ctrl-alt-+ (the + on the notepad), right? My suggestion when dealing with monitor/video card setup problems: Set the starting conditions to init 3 not init 5 to avoid getting X started. Then log into the console as root and use sax2 to configure what needs to be configured. This way you can be sure that a running X process is not interfering with what you're doing.
