What sort of keyboard is it? USB? Could it be that you have somehow removed hotplug from runlevel 5?
Standard 101, 102 and 105 keyboards all fail identically. Only USB devices I own are a couple of trackballs, none of which I use on this machine. chkconfig hotplug is 1235. Hmmmm. I just noticed that chkconfig kbd 12345. Everything else in 4 is off.
