Why don´t you try the MIMEDefang?
Mimedefang works at MTA level (sendmail) performing virus
scan, blocking atachments and spam (using SpamAssassin).

I´m using it at a Suse 8.0 w/ Sendmail as a Relay server.

You could have fetchmail pop your mail, deliever it to sendmail, which hands it off to procmail since the address is local, which pipes it through spamassassin on its way to your local mailbox, and use kmail to get your mail locally. I am doing this here, except I am using Netscape mail. YMMV
