The above two WARN_ON() are caused by bug in block layer timeout and should be
fixed by the following two commits.

Can you please try KOTD and see whether the problem is still there?

Although the above two commits fix the WARN_ON() and ensuing problems, they
don't fix the original timeout. Please try to burn with 'wodim -v' and post
the output of wodim and kernel log after failure.
